Stress Test
A Stress Test, also known as an Exercise Stress Test or Cardiac Stress Test, is a non-invasive diagnostic procedure used to evaluate how the heart functions during physical exertion. By monitoring the heart’s activity while the body is under stress, physicians can identify cardiovascular conditions that may not be apparent when the patient is at rest.
Stress testing plays an important role in the diagnosis, risk assessment, and management of heart disease.
Purpose of a Stress Test
A stress test is performed to:
assess blood flow to the heart muscle during exercise;
detect coronary artery disease;
evaluate heart rhythm abnormalities induced by exertion;
determine exercise tolerance and cardiovascular fitness;
investigate the cause of chest pain, shortness of breath, or fatigue;
assess the effectiveness of cardiac treatments or medications;
guide safe exercise and rehabilitation programs.
Types of Stress Tests
Depending on the patient’s condition and clinical goals, one or more of the following stress tests may be used:
Exercise Stress Test (Treadmill or Bicycle Test) – monitors heart activity during physical exercise;
Pharmacologic Stress Test – uses medication to simulate the effects of exercise in patients unable to exercise adequately;
Stress Echocardiography – combines stress testing with ultrasound imaging of the heart;
Nuclear Stress Test – evaluates blood flow to the heart using a small amount of radioactive tracer.
The type of test is selected by the physician based on the patient’s medical history and physical ability.
How the Test Is Performed
Electrodes are placed on the chest to monitor heart rhythm (ECG).
Blood pressure and heart rate are measured at rest.
The patient begins walking on a treadmill or pedaling a stationary bicycle, with intensity gradually increasing.
Heart rhythm, blood pressure, and symptoms are continuously monitored.
The test is stopped once a target heart rate is reached, symptoms develop, or adequate diagnostic information is obtained.
A recovery period follows, during which monitoring continues.
The test typically lasts 20–45 minutes, including preparation and recovery.
Benefits of a Stress Test
non-invasive and widely available;
provides valuable information about heart function under stress;
helps identify hidden or early-stage heart disease;
assists in treatment planning and risk stratification;
supports safe return to physical activity or exercise.
Preparation and Aftercare
Preparation:
Patients may be advised to avoid food, caffeine, or smoking for several hours before the test.
Comfortable clothing and athletic shoes are recommended.
Certain medications may need to be temporarily adjusted under physician guidance.
Aftercare:
Most patients can resume normal activities shortly after the test.
Temporary fatigue may occur following exercise.
Results are reviewed and explained by the physician.
Safety and Risks
Stress tests are generally safe and performed under close medical supervision. Rare risks may include:
chest pain or shortness of breath;
dizziness or fainting;
abnormal heart rhythms;
heart attack (very rare).
Emergency equipment and trained staff are always present to ensure patient safety.
Results
After the stress test, patients receive:
a detailed interpretation of ECG and imaging findings (if applicable);
assessment of exercise tolerance and cardiovascular risk;
recommendations for further testing or treatment, if necessary.
